Exposure & Response Prevention

ERP for OCD and Anxiety Disorders: The Gold Standard

If you’re struggling with obsessive-compulsive disorder (OCD) or an anxiety disorder and searching for effective therapy, you’ve probably heard of exposure therapy, or Exposure and Response Prevention (ERP). ERP is a highly effective, evidence-based approach that is considered the gold standard for treating OCD, phobias, panic attacks, health anxiety, and social anxiety.

How Does ERP Work?

ERP is a specific form of Cognitive Behavioral Therapy (CBT) that focuses on breaking the cycle of fear and avoidance. Here's how it works:

  1. Exposure:

    • Gradually face distressing situations, thoughts, or emotions. These exposures are done in a safe, controlled environment with your therapist’s guidance. The goal isn’t to scare you but to show you that you can face your fears without resorting to compulsions or avoidance.

  2. Response Prevention:

    • Response prevention involves resisting the urge to perform compulsive behaviors or avoidance strategies that temporarily relieve anxiety but ultimately keep you stuck long-term. This reduces your sensitivity to the trigger and eases anxiety over time.

How ERP Helps Specific Disorders

ERP is especially effective in treating a variety of anxiety and OCD-related disorders. Here’s how it works:

Obsessive Compulsive Disorder (OCD): For OCD, ERP helps you give up compulsions that only get in the way of living life freely. For example, if you experience taboo intrusive thoughts, you would learn to accept the thoughts instead of spending hours trying to suppress them. An exposure would be listening to an audio recording that triggers intrusive thoughts and images while being instructed on how to accept them instead of thought-stopping.

Panic Attacks: For panic disorder, it is common to avoid all anxious feelings for fear of them developing into a panic attack. ERP would use interoceptive techniques to help you learn that avoidance isn’t necessary to manage panic symptoms. An exposure would be breathing through a straw to simulate shortness of breath without trying to rush for anxiety medication such as Xanax, Ativan, or Klonopin.

Generalized Anxiety Disorder: ERP helps people with GAD put boundaries on worrying cycles. An exposure may be reading an article about a possible recession but instead of actively worrying and ruminating, you learn to mindfully place your attention on meaningful activities such as hobbies and leisure.

Phobias: Phobias often cause you to avoid situations where your fear might arise. With ERP, you would learn that safety behaviors and avoidance only prolong the intense distress you experience when triggered. If you are diagnosed with emetophobia, the exposure would be listening to hiccups without trying to stop the recording or by thinking, “good thoughts.”

Social Anxiety: In social situations, ERP helps you accept the anxiety without trying to suppress it. An exposure would be calling restaurants and asking if they have any reservations available without using apologies to feel less anxious.

Why is ERP Effective?

ERP helps retrain your brain to respond differently to anxiety-provoking situations. Repeated exposure and resisting avoidance behaviors teach you that compulsions aren’t necessary for safety. Imagine being able to touch a doorknob without immediately washing your hands or feeling an avalanche of anxiety. Imagine being able to let go of having a thought like, “What if I embarrassed myself earlier?” without requiring hours of internal debate. Research has consistently shown that ERP leads to long-lasting improvements in OCD and anxiety symptoms for intrusive thoughts, compulsions, and excessive worry.
